A conflict of interest arises when personal interests or involvement could impact business decisions or activities. A Service Provider Conflict of Interest may exist if any person in his or her official duties or having any role or responsibility that may involve in deciding or selecting could draw a connection or affirmation. This may be due to the possibility of influence in the performance of his or her official duties that may be damaging to the trust and confidence of the public and/or participant.
123 Support Services wishes to advise the Participant that is recommended that they find individual Service Providers for each category of Service they require. However, at 123 Support Services we understand the Participant has the choose to use 123 Support Services for more than one Service if they so choose.
Please fill out this Service Provider Conflict of Interest Declaration form as a manifestation that you believe that no Service Provider Conflict of Interest exists and wishes to sign on with 123 Support Service for one or more Services.
