Support coordination is a capacity building support which helps you to: - Understand and use you NDIS plan to pursue your goals; - Connect you with service providers, community, mainstream and other government services; - We communicate with you, your family and your providers to design a service plan which makes sure everyone can work together to help you pursue your goals; - Assist you in crisis resolution, helping you to resolve points of crisis.
